Hot springs or onsen (温泉) are naturally ocurring pools of warm water that players can find throughout the game. Resting at a hot spring heals William fully and bestows a buff that slowly restores his Life for 120 seconds. Hot Springs negate all status effects, including helpful ones like Divination.
Hot spring or Onsen are loved throughout Japan and have been so for the entirety of recorded history. The cleansing and purifying of the body through bathing is a tennant of Buddhism, yet Shinto and local folklore associate hot waters from the springs as gifts from the gods.
To this day, hot springs in Japan are considered beneficial for several pains, aches and ailments, based on the mineral composition of the water and the volcano it may originate from.

To achieve the trophy "Spa Healer", the player must enter an onsen for the first time. To unlock the trophy "Spa Lover", players need to find every onsen in the game. There are twenty in total, and they can be found in the following locations:
Kyushu Region

The Silver Mine Writhes | From the first shrine, head down the tunnel with the air purifier next to it. Hug the left wall until you find a ladder. Climb it and head right. You'll shortly come across another walkway below you. Drop down to find a hot spring inside the tunnel |
The Ocean Roars Again | From the shrine which is next to a ladder that leads to a bonefire, go straight and walk over a plank to enter a broken ship. Climb up the ladder and head left when you are on top of the roof. When you arrive the end of the next broken ship jump down on the right. Head right and follow the path to find the hot spring. |

- Partway through the game William ditches his boxers for a pair of traditional Japanese underwear called fundoshi
- Each hot spring is limited to one use. Exiting and re-entering a level will reset them

I don't know how, but I managed to get all 20 of them in order in my first playthrough of the game, without looking at any guides. Lucky me, I guess lol

does using hotsprings in other peoples worlds/yokai realm count towards the trophy?

In The SpiritStone Slumbers you can bring Ginchiyo with you to the Hot Springs and she has some reactions to seeing aswell as you using it in front of her
